- Please complete the following four forms. Do not forget to include your
child's name on each form.
- Have your therapist complete the Hometown Follow-up Therapist Form.
- Print out the Consent Form (Release Information), sign, and include your
$250 application fee payment (applied toward treatment if accepted into
program). We accept check, VISA, or MasterCard. To
pay with a credit card, please make sure you have completed the credit card
information at the bottom of the Registration Form.
